Metronidazole Induced Encephalopathy with Downbeat Nystagmus

Abstract
Metronidazole, a 5-nitroimidazole derivative penetrating the blood-brain barrier, has been used as an antibiotic, amebicide, or antiprotozoal drug. Metronidazole is usually considered relatively safe, but some serious adverse effects including peripheral neuropathy, seizure, and encephalopathy have been reported. We experienced a patient with pancreatic cancer presented with downbeat nystagmus, ataxic gait, and dysarthria after receiving metronidazole for the treatment of acute cholangitis. Although the true incidence and the exact mechanism of metronidazole-induced encephalopathy are still unknown, it can be encountered in clinical fields. Clinicians should be aware that an encephalopathy could be developed among patients taking metronidazole, and metronidazole might be one of the causes of downbeat nystagmus.
